What is PACE?

Our Program of All-Inclusive Care for the Elderly, also known as PACE, serves adults who qualify for nursing home care assistance, and older adults with physical, intellectual or developmental disabilities. PACE is a joint Medicare/Medicaid program that provides comprehensive medical and long-term care, as well as prescription drug coverage.

As a PACE member, you work with a team of dedicated healthcare professionals to coordinate all aspects of your care.

What Does PACE Provide?

The PACE Program provides access to medical care, prescription drug coverage, and long-term care supports and services without the worry of insurance bills, copays, deductibles or insurance gaps. 

Our care team works with you to develop a cost-effective care plan that includes approved paid services through our extensive provider network, as well as natural supports (like family members, friends and neighbors).

Am I Eligible for PACE?

To be eligible for PACE, you must meet the following requirements:

Age – Be at least 55 years old.

Residence – Live in one of the following counties: Kenosha, Milwaukee, Racine or Waukesha.

Long-Term Care Needs – Eligibility is based on the Wisconsin Adult Long-term Care Functional Screen.

Financial – Qualify for either Medicaid or Medicare; or be able to pay out-of-pocket costs.

  • Community Care arranges and pays for supports and services in your care plan.

  • There are NO copays or deductibles.

  • You MAY have to pay a monthly cost share to remain eligible for Medicaid, based on your income.

  • You MAY be required to pay monthly room and board.

  • You MAY be required to pay for care you receive from out-of-network providers, except in emergencies.

What Does PACE Cost?

Contact your county’s Aging and Disability Resource Center (ADRC) to learn about all long-term care programs available. Call Community Care toll-free at 833-900-PACE (833-900-7223) and we can answer any questions you have. Enrollment in any Wisconsin long-term care program is completed through the ADRC.
